    HMS Registration will take place at Hudson Middle School on the following dates:

    • 6th Grade               Wednesday, August 8 from 12-3:00pm or 5-7:00pm
    • 7th & 8th Grade     Thursday, August 9 from 12-3:00pm or 5-7:00pm

     

    REQUIRED ELECTRONIC FORMS

    Prior to Registration please log in to Final Forms  to update/confirm information and sign additional forms including:

    • Contact Information
    • Health History & Medical Profile
      • 7th Grade Immunization Form (Tdap & MCV4)

    REGISTRATION OVERVIEW:

    1. Enter through Commons:  Please complete all FINAL FORMS prior to Registration Day. At the Registration tables, you will receive schedules, handouts, student handbooks & locker assignments. 6th Graders (and new students) will receive their school lock and PE uniform.  Fees will be activated September 1 and are due by September 30.  
    2. Commons:  7th grade Vision Screening, Nutrition Services (lunch money collected) & various organization informational tables
    3. Media Center

      • PTO Supply Sale and Supply Lists
      • School pictures are taken (picture packets, including yearbook orders, will be available to complete at Registration). All students must have their picture taken, even if pictures are not purchased.  Picture and Yearbook Flyer
        Here are 3 options for ordering pictures and yearbooks:
         
        1. Online prior to picture day by going to mylifetouch.com (enter picture day ID of WP068002Y0 and your child's first name)  Simply select any of the portrait packages which include a yearbook (packages A-J). Or you may choose to build your own package but keep in mind that those do not include a yearbook.  
         
        2. You may order using the Lifetouch order envelope on picture day. Copies of those will be available at the school on August 8th and 9th. Yearbooks are included in each package (A-J) and there is also an option to order a yearbook alone on the flyer. 
         
        3. If your child is photographed but you do not order on or prior to picture day, you will receive a proof along with ordering instructions a few weeks after picture day. 
         
        By ordering online prior to and on picture day you can take advantage of many ordering options such as choosing a different background per page within your package.   Also you can sign up for the Lifetouch My Family Rewards program to begin receiving rewards for purchases. 
        New for 2018- receive a coupon for a complimentary 8x8 photo book from Shutterfly with any online order.
