What documentation is needed for children with allergy or asthma (symptoms) to attend school?

If a student has a known medical condition (such as allergies or asthma) with known associated symptoms (such as a runny nose or cough), parents can contact their healthcare provider and obtain medical documentation/doctor's note verifying the condition and associated symptoms. The documentation can be provided to the school clinic at the start of school, and then those students will not be sent home immediately if only those documented associated symptoms are present. Children will be sent home if any other symptoms are present: new onset cough or shortness of breath by themselves or at least 2 of the following: fever (100.4F or higher), chills, muscle pain, sore throat, loss of sense of smell or taste, and gastrointestinal symptoms of diarrhea, vomiting, or nausea.
